Tax returns for 2009 and 2010 (W-2 if 2010 return not available). If you can’t find return, tax transcripts can be obtained from the IRS at 1-800-829-1040. Have them fax it to you as if you request it to be mailed, it will likely never arrive. A fax is sent within 10 minutes but you must be near a fax machine as they will ask you if you are at the machine and won’t fax to an unattended machine.(privacy issues)
Paystubs(proof of pay or personal income) for the 6 months ending the month before we file. If you receive the same amount each paycheck all I need is the last 2 months. It is from this information that I have to prepare the Means Testing calculation under the 2005 law changes so it is very helpful to have detailed stubs showing taxes withheld and insurance etc. Self employed individuals please provide income and business expenses. Social security can be 2 months bank statements or annual award letter
A written or typed list of every creditor that you owe money to. This list should include names, addresses, account numbers and amounts owed to each creditor. In order to double check your records, free yearly credit reports are available at www.annualcreditreport.com. Please prepare a written list and don’t hand me the report as credit reports have a lot of useless information that is time consuming to plow through. Your list needs to include house and car creditors as well as any payday loans or medical bills also in addition to credit cards and school loans. Please use the inquiry or correspondence address if available since we want to get the creditor notified at the best possible address. I do not need phone numbers.
You will have to complete a credit counseling class as a requirement in order to file the case. It must be completed before the case can be filed. It is available online at www.freshstartonline.org or www.iflbk.org It costs $50. If you want to use our computer here at the office that is fine, however, it can be taken online anywhere. You will have to call them first at 866-662-4932 to set up a username and password. I can also sign you up after you come to the office. You will need to know your total unsecured debt amount (credit cards etc) and how much money you make working in order to do the class. We can not file the case without the Certificate of Completion of the course.
